The registration area is located in the main lobby of the hospital. Upon entering the lobby please check in at the information desk to sign in for your procedure. A hospital volunteer will direct you to the registration area.

Please remember to bring the following items with you at the time of registration:

• Picture ID
• All active insurance cards
• Living Will or Advanced Directives if you have one
• Any Doctors orders you may have
